Leadership

“I love working with our residents because I can make a difference in their daily lives. Whether the
individual is rehabbing to home, or a long-term resident needing assistance with their ADL’s, or a
resident transitioning to palliative or hospice care, I am deeply driven to ensure we make a positive
difference in the lives of our residents and their families.”
Brian Jaggers
LNHA, CDP
Administrator
With a calling to serve, Brian graduated from Eastern Kentucky University with a degree in Health Care Administration, taking his first skilled nursing facility administrator role shortly thereafter in 2001. Continuing his education while working, he earned his certification as a dementia practitioner, and joined Somerwoods in January 2009 to lead the facility’s diverse team of caring professionals as the facility administrator. He has been recognized by the Kentucky Association of Healthcare as Administrator of the Year and remains deeply committed to long term care and the local community.

“In many cases, seniors have a treasure-trove of stories and a wealth of knowledge to share— after working with seniors for an extended period, you can expect to look at life differently; talking with seniors about their lives might even change or reinforce your own life priorities.”
Pat Stratmann
RN, MSN
Director of Nursing
Knowing at the young age of eight that she wanted to become a nurse, Pat entered nursing school after graduating from Somerset High School, earning her ADN from Somerset Community College. Later she returned to school, earning her bachelor’s degree in nursing from Florida Hospital College of Health Sciences. She continued her educational journey, achieving a master’s in science from Grand Canyon University. For nearly three decades, she has served in many critical nursing roles, including serving as an emergency department nurse and House Supervisor of an over 300 bed regional hospital. With a passion to lead and a wealth of nursing knowledge in different settings, she joined Somerwoods Skilled Nursing and Rehab as the Director of Nursing in 2021 where she enjoys working with the clinical and front-line teams, while supporting residents and their families with kindness, care, and compassion.

“I love working with our residents and families and getting to know their personal stories.”
Craig Wesley
Admissions Director
With a passion to serve with over four years in the military including serving in Desert Storm, Craig leads each day with a people-centered mindset, working to make a positive difference in the lives of the residents, their families, and the team at Somerwoods. Serving as the Admissions Director for over five years, he is the first point of contact for families and residents— and ensures the onboarding process is full of love and support while reducing the administrative burden (whether for a short-term stay or senior transition to long-term care). During his time away from Somerwoods, Craig enjoys spending time with his wife and daughter and taking a spontaneous day trip to places they have never been too.
